NCMHCE practice cases, explained

The National Clinical Mental Health Counseling Examination (NCMHCE) is the clinical licensure exam most state boards require for the LPC or LMHC license. It is not a recall test. You read a client narrative, then make the same decisions a counselor makes in the room: what to ask, what to screen for, what the diagnosis is, what to do next, and what to avoid. PassReady Prep is built around that format.

How the NCMHCE is structured

The current exam, introduced in 2022, presents 11 case studies; 10 are scored and 1 is an unscored pretest case. Each case opens with an intake narrative and continues through clinical sessions, with 9 to 15 multiple-choice questions along the way. You get 225 minutes plus a scheduled 15-minute break. Questions fall across six content areas: Professional Practice and Ethics; Intake, Assessment, and Diagnosis; Areas of Clinical Focus; Treatment Planning; Counseling Skills and Interventions; and Core Counseling Attributes. Our case bank already includes items written to NBCC’s 2027 outline, so the practice stays current as the exam changes.

Why case simulations beat question banks

On the real exam, a confident wrong answer can hurt a client, and the scoring reflects that. Every decision in a PassReady Prep case is scored from −2 to +3, so your feedback shows more than right or wrong: it shows how close a near miss was and when a choice would have caused harm. Each rationale explains the clinical reasoning and names the trap, which is how you stop making the same category of mistake.

A study path that works

  1. Work one case a day, untimed. Read every rationale, including the ones you got right. Your first case is free and needs no account.
  2. Drill the weak domain. After three cases the study plan shows where you are losing points. Use the knowledge drill, decision trees, and DSM-5-TR reference to fix that domain before you move on.
  3. Watch the readiness score. It weights your accuracy by how heavily each domain counts on the exam, so it tracks the real test rather than a flat average.
  4. Sit a timed mock. When readiness holds above 60%, take the timed NCMHCE mock exam under the real clock and break schedule.
